Abstract Introduction Methylphenidate is a central nervous system stimulant used as first-line treatment for attention-deficit/hyperactivity disorder (ADHD). CNS use associated with increased risk of cardiovascular events such resting heart rate and blood pressure, sudden cardiac death, arrhythmia, stroke. Its safety profile in recipients transplants unknown, more research warranted to determine the adverse due medication this population. Methods Clinical case report, n=1 Results/Clinical Case A 19-year-old female history restrictive cardiomyopathy, arrest, orthotopic transplant has been treated methylphenidate ADHD approximately 2 years without incident. The patient was diagnosed between ages 8 10 historically medication. At age 13, she experienced arrest after volleyball game 4–6 minutes pulselessness.
